 Initial Visits:
We introduce people to Network Care and our office through a comprehensive initial visit. Your visit will start with a consultation with Dr. Piazza. He will take the time to find out what's happening in your body and your life and discuss with you your goals for care. He will work with you to Discover what you hope to achieve through Network Care. You will then receive a full spine and nerve system evaluation. This visit will be approximately 30-40 minutes.
On your second visit, Dr Piazza will review his findings from your initial visit, and share with you his best recommendations for care. You will then receive an entrainment and book subsequent visits based on the plan of care you discussed with Dr. Piazza. Including entrainment, this visit will be approximately 20-30 minutes.
 Initial Paperwork:
Before you arrive for your first visit, please download, print, and complete the appropriate documents below. Or, if you prefer, we can email them to you; simply contact us with your email address, and we will send them to you. Some of our questions may be different than what you may have experienced previously, so we really appreciate your taking the time and effort to complete these forms fully. This ensures that we will be able to best serve you in the office.

We have set aside special times for initial visits, separate from our normal hours of entrainment, so that we may best serve you.
